By submitting this form, I confirm that I have read, understood, and agreed to the following terms and conditions:

1. ASSUMPTION OF RISK.
I understand that working with animals carries inherent risk. I accept full responsibility for any injury or damage caused by my dog(s) to myself, others, animals, or property, during or after training services.

2. LIABILITY RELEASE.
I understand that Calamity K9 will take reasonable precautions to ensure safety during training. I agree not to hold Calamity K9, its trainers, assistants, or affiliates liable for any injury, loss, or damage that occurs during or as a result of training services.

3. HONEST DISCLOSURE.
I confirm that I have accurately disclosed all known behavioral, medical, and environmental information relevant to my dog(s). I understand that withholding or misrepresenting information may compromise safety and limit the success of training.

4. VACCINATION & HEALTH.
I certify that my dog(s) are current on core vaccinations (or have valid exemptions) and are not knowingly carrying a contagious illness. I will inform Calamity K9 promptly if health status changes.

5. TRAINING EXPECTATIONS.
I understand that Calamity K9 does not make guarantees regarding behavior outcomes. Training success depends on many factors, including genetics, environment, consistency, and handler follow-through. I understand that training is an ongoing process, not a one-time fix.

I acknowledge that Calamity K9 does not provide service dog certifications or legal documentation. Public access or task training services are only available to clients with qualifying disabilities under ADA guidelines. If applicable, therapy dog preparation will be discussed separately from public access or service dog protocols.

6. MULTI-DOG TRAINING
If I am enrolling multiple dogs, I understand that training time and structure may need to be divided among them or scheduled separately. Additional dogs may incur separate fees depending on the nature and goals of training.

7. CANCELLATIONS & RESCHEDULING.
I agree to provide at least 24 hours’ notice for any cancellations or reschedules. Cancellations made with less than 24 hours’ notice may result in a cancellation fee. Repeated short-notice cancellations may result in forfeiture of session fees or termination of services.

8. PAYMENT & REFUNDS.
Payment is due no later than 24 hours before the start of each session or package. Refunds are not guaranteed and are considered on a case-by-case basis.

9. DISCOUNTS & PACKAGES
Any discounted rates or session packages are offered at the trainer’s discretion and may vary based on the number of dogs, behavioral needs, and service structure. Package pricing must be confirmed in writing. Unless otherwise agreed upon, packages must be paid in full before the first session and used within 120 days of purchase.

10. EMERGENCY VETERINARY CARE.
In the event of an emergency during a training session where I cannot be reached, I authorize Calamity K9 to seek veterinary care for my dog(s). I accept full financial responsibility for any resulting treatment.
